Edit: I actually think it might be lateral displacement....help! Updated pic in comments.

Appointment on Monday.  I am only 4.5 months out so can’t imagine I did anything to cause this?  My right implant stays by my sternum when I lay down but my left slides away and leave a large dent.  From all my research this requires another surgery but I don’t want different implants and since I can’t see how this is my fault, am I going to have to pay again?!?  I am stressing big time.  When you pay this much money you just don’t want to be in the percent that has problems.

  • I don’t think this is bottoming out. Your nipple will point upwards and your incision can be visible and up on the breast instead of under the fold . This is what bottoming out looked like for me. How far out are you from surgery?  It could just be some late settling. I’m not an expert but I know what it was like for me!

  • You are not bottoming out.  I have the same thing happening with my right breast.  One is just dropping and fluffing quicker than the other.  They will be perfect in due time 😊

  • I hate to sound like one of those crazy ladies but something just seems different about it I guess.  I attached a laying down pic.  The one in question slides way more towards my armpit and make my sternum on that side much more flat.  My ugly sternum tattoo is mid chest for the most part and my right moves towards my pit but doesn’t leave me with a flat spot.  Is it because that one is higher and tighter and the left is actually the good one?!  I don’t know why I am stressing.  I am 4.5 months post op.

      JJB this is exactly how mine Look now when I lay down. From my research, I am %100 convinced it’s lateral displacement.

  • Hi there!  You're not bottomed out.  I agree you could have some lateral displacement.  Hard to tell with you right arm up.  Natural breasts fall to the side when you lie down so it's not abnormal but can be frustrating especially if asymmetric.  Treatment is 'lateral capsulorrhaphy'--to sew in or 'hem' the outside aspect of the pocket to make it smaller.  Commonly done with nearly every implant exchange unless going up in size as it happens to everyone to some degree.  Best of luck!!

      Dr. Roughton hi there.  Thanks for your response.  After some digging it does seem like lateral displacement.  My right implant is up by my sternum when I lay down but my left slides away from my sternum leaving a big dent looking area in my chest.  I am only 4.5 months out so would this likely be that my pocket was made a bit too large?  I can’t imagine that in four months my skin or muscles or tissues stretched that much?  I don’t want a different sized implant or anything but now am worried I will have to go back in and incur the cost of another surgery when this was out of my control? I made an appointment but just want to know what I am in for here.

    • JJB so every surgeon is different with costs for revisions as well.  At our practice we have something called revision assistance, a nominal cost up front that protects again the unexpected high fees for reoperation.  I think you have a good case for just asking if the pocket could be too big and what would that look like to revise it.  No harm in asking if that should incur a charge.  My sense is that the surgeon's fee will be waived.

      Dr. Roughton so he didn’t think it was displacement at all.  He said he just thinks my pec muscle was thinner in the area I feel the flat spot right by my sternum.  He said he could take some fat from my love handle area and essentially inject it in to “soften that area” and give it more of a curve.  Is that a legit thing?!

      JJB yes, that's definitely a thing and pretty common from what I've seen. It's called Fat Transfer and pretty noninvasive, I believe. Look into it to be sure, but totally a thing.

      I was looking at the pics and to me and my untrained, uneducated opinion...to me it looks like the issue isnt between your breasts but the other side of the "sliding" boob. I've seen situations very Similar where the pocket separates from the chest wall and/or muscle causing the implant to slip outside of the pocket area and down more into the armpit side. Unless you're lying down, it's not really detectable but as soon as you lay down, that one implant goes on a journey. Sound like your situation, or not really? 

      If so, the way I've heard to help correct this is 1) your PS has to go in and close that section where the pocket is compromised. I think they just stitch it up with dissolvable stitches. I believe they may either stitch it back up against the chest wall or to the muscle, not sure. Then 2) the patient wears a compression strap while it heals and the pocket takes shape again. 

      Problem solved! But that's only IF that's what your situation is. If he's just shrugging it off and suggesting fat transfer, if it were me I'd get a 2nd opinion. Maybe find a different Surgeon nearby at a completely different place and have then assess the situation. Then....see what they think. They may say the same thing as your PS but at least then youd have more confirmation before electing for a new procedure. I just dont understand it enough to see how a fat transfer would keep the implant from moving over there. To me seems like support needs to come from the other side to push it back to center?! But what do I know lol

      Do me a huge favor in order to give you my best educated opinion.  Have someone else take your photo straight on and with the 'flat' breast to the side and post them.  That will take the arm effect out of it.

      Dr. Roughton please let me know if these are helpful.  If not we can take others using daylight so there are no shadows.  The boob in question is the same side as my rib cage/side tattoo.  There are two side views of that one.  One with my arm to my side and then there is a picture of each with my arms out to a “T”.  What do you think? I added one more that is closer up while I am on my back and circled where there is a visible divet when I look down.

    • JJB does this happen when you are standing?  Do you have a preop photo?  I ask because if your “affected side” (I get confused with the mirror effect) had a nipple more to the outside/lateral than the other side then the doc just centered the implant under the nipple and that’s the right thing to do I think. If it only happens when laying down and you can move it over (medial or towards breast bone) then I think it’s laterally displaced and no amount of fat grafting will totally correct it.

      Dr. Roughton they seem pretty centered and the same to me.  I can definitely push move it towards the center while laying down.  Here is a frontal picture with my arms down and one of the only pre op photos I can find!  My left has definitely dropped more than my right but I think while standing they look pretty much like they are both facing forwards? Affected side is the side with my rib tattoo.  It’s opposite is the pre op pic because I took that one and my husband took the other frontal one.  It’s the boob opposite of my half sleeve.

    • JJB in my opinion it’s lateral displacement. Would respond to lateral capsulorrhaphy.  Looks normal standing and falls to the side lying down. Now your surgeon may see something different in person. 🤷🏼‍♀️

      Dr. Roughton no thank you!  You’ve been so helpful!  I am just concerned because I felt as if he sort of brushed me off and just said it’s just the way my anatomy is and that fat grafting or a larger implant would fix it.  I definitely don’t want bigger implants.  Is it acceptable to go to someone else for a second opinion and then go back to him with said opinion?  I don’t want to insult him and normally I am a very assertive person but just feel so weird about approaching this especially now that he brushed me off once.

      JJB I felt EXACTLY like you. I went to my surgeon he suggested the fat transfer. I don't have a lot of fat to lipo out, and on top of that I just don't want to get liposuction. But I also felt like I got blown off, I also am a normally assertive person and felt uncomfortable speaking up against my doctor. Please let us know how the second opinion went. I'm trying to make myself go get a second opinion as well.

      Daj I am getting my second opinion mid August after my husband and I go on vacation.  I am 100% convinces it’s lateral displacement and not just “my skin being thinner in that area.”  Once I have that confirmation, then I just need to decide if it bothers me enough to actually be much more assertive with my surgeon and try to get it fixed.  Did your PS also suggest fat grafting?

      JJB He said that the only option he could think of was that. I just don't think I have enough fat to lipo and also I don't want to deal with complications and recovery, or any weird changes in my body from liposuction that I don't need.
